I have been involved in ordering a couple of hundred of these. To answer your questions The usual procedure is to send just the the part of the CMSR's that confirm the veterans service. In recent years you can get by with sending the index number of the veteran as it is listed in the National Parks Service Civil War Soldiers and Sailors List.

In this Soldier and Sailors list each regiment has an enumeration of all soldiers in the regiment listed and numbered alphabetically. That is the number that I send.

As for the Social Security and Service Number usually those are left blank for Civil War veterans. I suppose that you could put the Soldiers and Sailors number there or the CMSR Microfilm roll identification number there. But I usually place that information at the bottom of the form in the Additional information Block to avoid confusion.

A word of warning. It is very difficult to obtain a memorial stone. You will need to provide information that the veteran is Not buried in some other place that is marked such as a mass grave on a National Battlefield. Usually that information is pretty hard to document.
