It probably came from his CSR. These can be seen on Fold3.com. Back in late 1800's clerks began recording information from records of the war onto individual soldier compiled service records. They would take hospital ledgers, company morning reports etc record the info on a separate paper and place that info in a soldiers CSR. That is where almost all of the info came from on the rosters and info on the site.
