Cav.]** November 5, 1863, one horse appraised at $700, wounded at Resaca May 14, 1864, right leg amputated the following day, applied for an artificial limb November 2, 1864 at Forsyth, Ga., resident of Cleveland, Tenn., appears on a Register of Beneficiaries of the Association for the Relief of Maimed Soldier, Richmond, Va., no date

** SO #8 dated Richmond, Jan. 11, 1865 direct that “Ten of the eleven companies composing the organization known in the Army of Tennessee as the [Avery’s] 4th Georgia Cavalry (formed upon the basis of the 23rd Geo. Battalion) will constitute the 12th Reg’t. Geo. Cav’y to take effect from the 30th Jan’y, 1863.
