I had read through this online presentation previous to posting but failed to connect all the dots.

Perhaps John T. Bazar was simply applying for residence in the Oklahoma Confederate Home at Ardmore and the documentation request went to Dr. Hailey at his residence/place of business in McAlester. The correspondence between the Adjutant General's office and the President of the Trustees of the Confederate Home is dated 9 JAN 1915.
