I have never seen one and to get one you will probably have to work through all of their Compiled Service Records by regiment to get that information.

It is possible that such records are also at the National Archives in Washington DC in the files for each regiment.

Lastly, checking period KY newspapers often yields such information as well. I have seen a lot of casualty lists in these papers over the years.
