I would guess that the supposed female lieutenant actually was a vivandiere. Some Louisiana regiments followed the French custom of allowing women to serve in the regiment and they had their own distinctive uniforms. The man probably assumed the uniformed woman was an officer. The famous photo of Coppens' Battalion at Pensacola has a vivandiere on the left of the line in uniform (with plumed hat and sword if I remember correctly).
