Here's what's in the book. There are two Blankenships in this company, Joseph A. and David. Joseph was 17 when enlisted; David was 20. Both were enrolled in Madison County NC and may have been from there.

The J. W. Blankenship reference in the index is probably a reference card, and means that at least one of the cards or documents in Joseph Blankenship's service file used the middle initial 'W' instead of 'A'. Without the man's signature, I'm not sure how to say which one is correct.
