"under God" was added in the 50's during the Eisenhower administration. I learned it without the phrase but with nuns for teachers, we quickly learned the new version.
Although I don't remember it, I am told when my mother was in school, they extended their arms toward the flag rather than placing the hand over the heart. The arm salute was too close to a fascist salute, I was told.
