I'm sure it wasn't just one reason for Lee to be so far removed from his normal 'common sense'. The problems you mentioned could certainly been part of his temperment at that time. It's just always puzzled me that the one error or maybe less than well thought out call he made was so devastating to those not only involved but those who watched in helpless positions in the area. The outcome was pretty clear barring a miricle and still, he carried through.
