I've read they stored ice in cellars or something and insulated it with hay or something.

Dr. Gorrie (from Apalachicola) had invented an ice maker before the war. Neat museum in Apalachicola at his house. The reason he tried to even come up with one was in hopes it would help save or at least help patients during the pretty common epidemics of Malaria and Yellow Fever.
