Combined Service Records

Where and when were Combined Service Record cards created? I have the set for Joseph Phipps of the 18th Texas Cavalry. Some of the cards have the source (e.g. Roll dated Feb 8, 1863), others don't. Some have roll and sheet numbers at the bottom.

All have copyist signatures. One indicates he was killed in battle Sept 20, 1863 at Chickamauga. Someone by the same name and death date is buried at Chattanooga National Cemetery where Union soldiers killed at Chickamauga were buried. The VA at Chattanooga assure me only Union soldiers are buried there.

It seems unusual for two men of the same name to be killed in the same battle on the same day, but serving on different sides.

Could the card have been erroneosly prepared with info on the grave for some other Joseph Phipps?

I presume those items sourced from rolls could be verified from rolls, probably at NARA, but this one doesn't give a source.
