Someone is probably going to make a liar out of me, but I don't recall any testimony of the Joint Committee re The Crater. The Committee was such a political body, and spent much of its time investigating officers with Democratic Party leanings. Since the principals involved in the Union fiasco at The Crater generally fit the political tilt of the Committee, I suspect that there was little interest in launching a full-scale investigation of the causes for the fialure there. And its a shame, since it was an excellent plan, which had a real prospect of breaking Lee's front, but any chance for success was thrown away by the mishandling of troops by Federal tactical commanders. Or at least that's my take on it.

I've always read that Ferrero was drunk at the time of the attack. But drunk or sober, he had not forged much of a commendable combat record.
