I need some help on filling out the forms to order Confederate VA headstones for cemetery projects I'm working on here in Chilton Co., Alabama.

I have three burial stones and one memorial stone to order for the first cemetery we're doing.

My questions are:

I've been told that the CMSR's are the type of documentation that I can send with the form. Do I need to send everything in the CMSR or is there one particular record that I need to send.

It asks for SSN or service number... of course there is no SSN for these men but is there a service number? Do I just leave this blank for CSA stones?

Most of the stones I'll be ordering will be the flat marble. What is the emblem code for the Southern Cross of Honor? I can't seem to find that.

Do they take information like "25th AL INF Co C" off the CMSR's or is there someplace to write it in that I'm not seeing.

Thank you ... I have many CSA veterans I'll be doing this for and I need to get this system down pat.
