In some 20 years perusing soldier’s service records, I’ve never seen a document such as Quarterly Returns of Deceased Soldiers, nor a reference card containing this data in any one CSMR.

Why a war department clerk copied the data for one company into the records of one soldier, had to have been an error, as only the data for that individual should have been recorded.

It is not believed any of these company quarterly reports have ever been microfilmed except those for the 54th Massachusetts Inf. (Colored), and available at Fold.3 https://www.fold3.com/image/296974658

I suspect however, that on a visit to the National Archives Old Army Archivists on Pennsylvania Ave. in Washington, D. C., you could inspect selected copies.
