There are numerous incidents in which the Broadfoot index is wrong. After all, it is a copy of a copy of a copy, three levels removed from the original rolls:

4) Broadfoot -------------- copy of NARA folder index,
3) NARA folder index ---- copied from NARA compiled service record cards
2) NARA CSR cards ----- copied from original rolls.
1) Original rolls.

The original rolls had multiple names on these lists, so the NARA clerical staff produced individual cards for each entry and filed them with the CSR's. CSR's also include reference slips for some documents which refer to more than one individual, but no cards were produced. Of course some original records appear in the CSR's which refer solely to individual soldiers.
