I searched Soldiers and Sailors System, as well as Civilwardata.com, and also came up blank on an officer A.Kelly in the PA 33 or the PA 4th Res. Could this be an early example of provenance being "added" to a weapon post-conflict?

AS for the book, go to interlibrary loan at the reserve desk of your local library. They can send a query. They may only find it in a reserve section, but at least you can travel to that library to peruse, or query that library's staff to check the book's index for Kelly.
