Watkins died soon after the amputation of his leg in Hagerstown.

His grave appears on a post-war inventory of the Confederate dead scattered about Washington and Frederick Counties. He was originally interrred in the Washington County Almshouse Cemetery located on North Locust Street in Hagerstown. He is listed there as H. Madison Watkins. This cemetery was used to inter the soldiers of both sides who died in town throughout the war. By the time the war was over, there were nearly 3 dozen Confederates and over 200 union soldiers buried here. The Union soldiers were reinterred at Antiteam National Cemetery in 1867. The Confederates were reinterred in 1874 at Washington Confederate Cemetery, located within the confines of Rose Hill Cemetery on South Potomac Street in Hagerstown.

Presuming he wasn't missed when the Confederates were reburied in 1874, Watkins lies in an unmarked grave in Washington Confederate Cemetery.
