Mary K. Whittington stated in her application that her husband served:

1. in the Washington Artillery, 5th Company, as 1st Lieut. (the records show 3rd Company)
2. in the 2nd Louisiana Cavalry, Company H, as Captain, she mentions Col. Vincent and Lt. Col. J. D. Blair
3. two months before the surrender promoted to Adj. General, staff of Brig. General Brent

She lists these witnesses:

Captain Jumel, 2nd La. Cavalry, Baton Rouge, Louisiana
Captain E. T. Lewis, 2nd La. Cavalry, Opelousas, Louisiana
Louis Riche, Co. H, 2nd La. Cavalry, Plattenville, Louisiana

Included in the file is an affidavit from Captain E. T. Lewis.
