As Ron explained most young men went north for formal education. Unfortunately many men practiced medicine without the benefit of a degree. Any physician who entered the Confederate military did so voluntarily, but normally had to pass a formal examination to be commissioned as Surgeon or Assistant Surgeon. Someone had to deliver babies, set broken limbs and care for the sick at home, so not every doctor joined the army.
