Alan and Greg are both right - having just gone through a ton of Tennessee wartime newspapers for another research project, I made note of all the articles on the Donelson POWs and what happened to them.

5 steamboats plied the river between Nashville, Clarksville and Dover/Ft. Donelson. One boat even came in on the morning of the 16th to drop off troops just in time to surrender!

The Confederate breakout attempt was over by 3 PM on the 15th so anyone wounded would have had ample time to be placed on one of the boats for evacuation. One of the articles I found mentioned boats taking troops out of Dover/Donelson. I suspect this is how he got out but we cannot rule out that he went out with Forrest either.

The wounded that did get out on the boats went to Clarksville or Nashville with some being captured there when those cities fell not long after Donelson did.
