The CSA Medical Service only had two ranks. Ast. Surgeon and Surgeon. The Ast. Surgeon was equivalent to that of Capt. A Surgeon was equivalent to that of Major. Few Surgeon's and most Medical officers attached to field units and in hospitals were Ast. Surgeons. My G G Granduncle graduated from U of Penn in 1854 and started practicing medicine. He joined the CSA Medical service as an "acting" Ast. Surgeon (civilian contractor) and in early 1862 was made and official Ast. Surgeon. He was assigned to hospitals in VA (his home state) until he was transfered to the Army of Tenn., 40th Alabama Inf. in late 1863. He stayed with this unit until after the fall of Atlanta when he was transfered back to Army of N. Virginia, 38th GA Infantry, still an Ast. Surgeon. He was not promoted to Surgeon / Major until April 1, 1865.
