Thanks for the information. I talked to one of the archivists at LSU-S Noel Memorial Library Special Collections and they do have a copy and are sending me a reproduction. The document is a spiral bound copy of the W T Block transcription, with 34-35 pages of text. There are one or two pages of CSR included.

It clearly is an unpublished document, with 4 libraries (UTx Austin, Houston City Library, LSU-S Archives, & Sam Houston State Un. library) holding donated copies. I am still hoping to ID the location and holder of the original, for citation purposes.
